Heading Home Hennepin is the community plan to end homelessness in Minneapolis and Hennepin County over ten years. The plan has specific strategies in place to prevent homelessness whenever possible, provide comprehensive street outreach, create thousands of new housing options, and connect people to the services they need to escape homelessness.
